Evaluate your readiness for a QCTO site visit with this comprehensive checklist
Instructions
Answer "Yes" or "No" to the following questions. Each "No" indicates a critical area that requires immediate attention before a QCTO site visit.
Your Information
1. Legal & Administrative Compliance
Is your provider legally registered (CIPC, EMIS, HEMIS, etc.) and do you have a valid Tax Compliance PIN (or SARS exemption certificate) readily available?
2. Physical Premises
Do you have valid, signed proof of ownership or a legally binding lease agreement for your training premises that covers the entire proposed accreditation period?
3. Human Resources & Management
Do you have a detailed organogram with clear reporting lines and signed employment contracts for all facilitators who will be present on the day of the site visit?
4. Facilitator Qualifications
Do all your facilitators have certified copies of their IDs, qualifications, and CVs on file, and are their qualifications and industry experience directly aligned to the curriculum you are applying for? (For non-SA facilitators, do you have SAQA evaluations, work permits, and valid visas?)
5. Learning Resources (Knowledge & Practical)
Do you have a completed inventory list of all physical resources (e.g., chairs, projectors, tools, equipment, consumables) that exactly match the requirements listed in the official QCTO curriculum document?
6. Learning Material
Is your learning material (e.g., manuals, guides) fully developed and can you clearly demonstrate how it covers 100% of the knowledge, practical, and workplace competencies outlined in the curriculum?
7. Workplace Experience Component
Do you have a signed Memorandum of Understanding (MoU) or Service Level Agreement with an industry partner for learner placements, and a detailed plan (including logbooks and a monitoring schedule) for managing and assessing the Workplace Module?
8. Online/Digital Delivery Readiness (If applicable)
If offering online learning, do you have a licensed Learning Management System (LMS), valid software licenses, a reliable internet contract, and a documented process for delivery, assessment, and learner support online?
9. Policies and Procedures
Do you have implemented, written policies for: Quality Assurance, Assessment, Appeals, Occupational Health & Safety (OHS), and Learner Support?
10. Occupational Health & Safety (OHS)
Has your venue had a recent OHS audit? Are fire extinguishers serviced, emergency exits clear and marked, first aid kits fully stocked, and OHS representatives appointed and their details visibly displayed?

9-10 "Yes" Answers
You appear to be highly prepared. Conduct a final internal review against the full Form 5 checklist to ensure no details are missed.
6-8 "Yes" Answers
You are on the right path but have significant gaps. Prioritize addressing the "No" answers immediately.
Fewer than 6 "Yes" Answers
You are not yet ready for a site visit. Use the attached QCTO forms (especially Form 5) as a blueprint to build your systems and gather the necessary evidence before applying.
